Dan Forman, MBA, ACHE, Executive Director, has held several executive positions within the Townsend treatment system over the last decade including roles as Vice President of Marketing and Director of Business Development. His primary goal as Executive Director is to deliver an exceptional treatment experience to patients through unparalleled service and support.
Born and raised in New Orleans, Dan is very active in the community. He serves on many nonprofit boards and is involved in various community organizations, civic associations, and industry affiliations. Highlights include his role as president of the Young Leadership Council, the largest member-driven nonprofit in New Orleans. He is also an active member of the American College of Healthcare Executives (ACHE).
Dan received his MBA in business administration from William Carey University, and his bachelor of science in communication from Boston University. Other educational highlights include being selected as an inaugural fellow for the Norman C. Francis Leadership Institute at Xavier University, which educates and prepares professional men and women for consequential civic engagement and purposeful social responsibility in the New Orleans area.
Dan lives in New Orleans with his wife and three sons. He enjoys international travel, Broadway musicals, and live concerts with his family.
Susan Julius, MD, ABAM, ABFM, AAMRO, Medical Director, is a clinician, speaker, and teacher. She is board certified in Family and Addiction Medicine, as well as Physical Therapy, and holds a special degree in urine drug testing analysis. Dr. Julius studied Physical Therapy at Cleveland State University and attended Case Western Reserve University School of Medicine, in Cleveland, Ohio. She trained for two years in General Surgery, at Tulane University School of Medicine in New Orleans, Louisiana and completed a Family Practice residency at Ochsner Foundation Hospital, in Jefferson, Louisiana.
Dr. Julius incorporated her background in these areas, along with her understanding of the biology of addiction and her personal journey in recovery, to treat patients and families with this disease for over ten years. She worked at Choices of Louisiana, a Methadone clinic, in La Place, Louisiana and was Medical Director for the City of New Orleans Homeless Clinic, and Townsend Clinic, in Lake Charles and Baton Rouge. Currently, she is the Medical Director at Townsend Recovery Center, in New Orleans.
Dr. Julius has been an expert consultant and keynote speaker for American Addiction Centers and was featured on NBC News, Fox News, Baton Rouge Business Report, and New Orleans Happy Hour Podcast. She worked with the New Orleans Re-entry Program and is active in AA and served on the Local Action Committee for International Doctors in Alcoholics Anonymous. She is a member of the American Society of Addiction Medicine, American Academy of Family Physicians, the American Medical Association, and the American Association of Medical Review Officers. She lives in New Orleans with her husband and three dogs, and loves to swim, garden and operate her vacation rental.
Jacob Goldberg, MSW, LMSW, LAC, CCS, Clinical Director, is a hands-on leader and innovator who takes personal satisfaction in program development, training, and problem solving. As Clinical Director, he has a comprehensive understanding of healthcare and provides guidance and vision through strategic planning, organization, and supervision. Jacob has over 10 years of experience in the private addiction field serving adults, family members, and loved ones with mental illness, substance use disorders, and chronic co-occurring conditions. Jacob received a bachelor’s degree in social work (BSW) from Southeastern Louisiana University and a master’s degree in social work (MSW) from Louisiana State University. He is a Licensed Addiction Counselor (LAC), a Certified Clinical Supervisor (CCS), and Licensed Master Social Worker (LMSW). Jacob has a passion for the outdoors and fitness. He currently holds an OUPV Captains license through the United States Coast Guard and enjoys cooking and spending time with his friends and family. Jacob understands the complexities of Addiction and is committed in his efforts to help our patients gain knowledge of this disease and navigate their own personal recovery.
Tori Hatter-Smith, MS, LAC, PLPC, BC-TMC, Counselor, is a professional counselor in both the mental health and drug and alcohol recovery field. She holds a bachelor’s degree from Southern University in speech pathology & audiology and a master’s degree in human services with a concentration in mental health counseling from Capello University. Tori is a Licensed Addiction Counselor and a Provisional Licensed Professional Counselor. She has worked extensively in both the public and private sectors. She has worked for the past 18 years providing direct service counseling and supervision. She has provided services for the inpatient and outpatient recovery program of Townsend Recovery Center as well as other services organizations and programs. Tori has a broad range of clinical skills including individual, group, and family recovery counseling. She provides confidential services in the privacy of an office setting and assists patients and families in fostering the first steps to consider when addressing issues related to addiction, as well as ongoing recovery treatment. Tori believes that all people, if given a chance within a supportive environment, can change and move successfully towards a healthier lifestyle.
Nalin Patel, CIT, is currently in school working toward his degree in social work. Throughout Nalin’s career, he has worked in a hospital setting, and since entering recovery himself, has taken his gift to the field of recovery. He enjoys cycling and spending time with his dog, Pepper.
Anthony Thomas, BS, Behavioral Health Technician Manager, received his B.S. from Southeastern Louisiana University. He has over 10 years of experience in working with the dual diagnosis and mental health population. He worked as a mental health counselor for six years at River Oaks Hospital and was a mental health case manager for three years at Dallas METROCARE Services. Anthony utilizes his experience to help motivate, encourage, and guide others in finding their path to permanent sobriety. Anthony enjoys spending time with his wife and children and staying in shape at the gym.
